Anomalous Junction of pancreaticobiliary Ductal System(AJPBDS) in Adult Choledochal Cysts


Abstract
Choledochal cyst is a relatively uncommon disorder, but it occurs more frequently in orients (especially in Japan) than in the united states of America or Europe.
In recent years, it was found that choledochal cyst is often complicated with Anomalous Junction of Pancreaticobiliary Ductal System (AJPBDS) by Babbit.
AHPBDS was divided into two subtypes by Kimurat type I (pancreaticocholedochal type), type II (choledochopancreatic type).
We have done retrospective studies on the subtypes of AJPBDS in adult choledochal cysts. The total number of choledochal cysts were 22 cases in adult and 18 cases in child. We exclded 23 cases that were not performed ERCP or PTC (including
choledochal
cysts in child). therfore studied 17 cases with ERCP or PTC.
@ES The results were as follows:
@EN 1) The male to female ratio was 13.6%.
2) The average age was 30.6 years.
3) The ratio of AJPBDS was 76.4% (13 cases); tyep I (11.7%, 2 cases), type II (64.7%, 11 cases).
4) Tdhe average lenght of the common channel was 45 mm in type I, 24.5 mm in ytpe II.
5) The combined diseases with AJPBDS were gall stones (5 cases, 29.4%), adenocarcinoma arising from choledochal cyst (I case) and they occurred all type II AJPBDS.
6) The operations for choledochal cysts were changed from cystoenteric anastomoses (9 cases, 40.9%) to cyst excision with nepaticojejunostomy (13 cases, 59.1%).
7) The post-op. complications were pancreaticocutaneous fistula (1 cases), post-op. bleeding (1 case).
